ESSS Child Self-Report Survey

In accordance with North Carolina SL 2023-106, the Parent Bill of Rights, the full text of our child self-report surveys is provided below. Please note that participating schools in North Carolina will only administer sections 4 and 5, after the initial questions on page 1. Also note that these instruments were designed by professionals for use with children and were approved by the Institutional Review Board at East Carolina University (UMCIRB 21-001728) for this project. Parents who have questions or concerns are encouraged to contact the project coordinator, Allison Dembowski at (252)328-1377, or an administrator at their child’s school.
