MAPS

The Person-Centered Planning process is a way for the student, families, educational team, and community members to plan for the future of the student. A variety of maps are created by participants to share and plan for future goals. Participants develop future goals and an action plan during a collaborative process in which they share information based on their experiences and knowledge of the student. Future goals are identified and prioritized then lead to the development of an action plan. Team members share common knowledge about the student.

The general process consists of the following steps:

  1. Family identifies their preferences for meeting participants
  2. Identify the facilitator
  3. Invite the participants
  4. Determine a meeting location and time
  5. Complete a series of maps that might include student’s history, strengths, likes and dislikes, dreams and fears, communication, ideal day, typical day, etc.
  6. Prioritize needs and develop goals
  7. Develop an action plan for the goals
  8. Monitor progress and have follow-up sessions
